Please do not send your loved one here. Therapy services are hit or miss depending if they like the patient. Staff is stretched too thin and the good ones are burned out. "Lip service" is given to concerns or complaints but nothing is changed. They have no training or support to handle patients with medical issues. More concerned with the outward appearance than fixing the underlying issues of staffing levels, training, communication, and patient care.

If I could give this place a negative I would. My mother was here less than a week and the negligence of the LPN Shayquell Mitchell was disheartening. When the first incident was reported to Stacy Bobo, Director of Nursing, I was unaware they were friends and left Millennium to come over to Opus (they are sister companies). Which is why the incident was knocked so lightly. After the second incident, it was reported Joseph Hanna, the administrator, and he too has been negligent by not replying and allowing this behavior. Keep your love ones away from here! The staff is incompetent, unbothered, and very rude!

Horrible! Do not send your love one there! My loved one was there and on two instances, this LPN Shayquell Mitchell was taking of my her and was negligent! We report her the first time and nothing happened! Reported her to the head of the facility and still nothing! Something needs to be done to the place!!!

The way they treat their staff is horrible, the ones who have been there longer are always right and everyone else is wrong.... Its just horrible, they talk down to them like they're children, the upper management team needs to be replaced immediately!

85% of the staff clearly show up to work for the pay check. Forget about patient care.
The other 15% of staff (mainly therapists), are great.
Impossible to get a drink for the resident, clean linens, rooms do not even have wash clothes or towels. Good luck cloth bathing.
Call light button for nurse/cna/ receptionist station stays on for extended periods of time.
Absolutely would not waste a family members days on this earth at this facility.
