This place doesn't even deserve a star. Horrible!! They lied about giving my grandmother a bath i came two days in a row and the second day they said she had just had a bath but was still wearing the same clothes she was in the day before with dried pee on them!! They new she had a whole closet full of clothes. I had to have them bring me a shower chair and I went and cleaned her myself. When in the shower room it was disgusting dirty diapers on the floor. One morning my dad came to check on her and her oxygen nose piece was on the floor!!! She was in there for about three days and we realized she wasn't getting her proper medicine she couldn't even talk brought her home and gave her the right stuff she was able to go to the bathroom on her own. I hope this post helps you. If we didn't get her out when we did she wouldn't have lived as long as she did. This place really needs to be shut down.

Staff takes food and money from patients and have known threats from staff. If we only knew what we know now.

This place shouldn't even get a star! Horrible!!! Everything about this place is horrible. My dad had to come here after having a really bad stroke. Not even 2 hours of him being here he was left unattended he fell and busted his face open! He had to go back to the ER to get stitches! We did ALL OF his care!! The staff we're very inattentive. They would forget about his meals so we had to bring him food. They checked on him only once a day. If you question anything they did they we're rude. You can really see that no one cares at this place. My family had to take shifts because we were afraid to leave him alone with these people! I wouldn't recommend this place to anyone!!!

Totally understaffed. My brother got bed sores because they didn't rotate him. The nurses light stayed on for hours. The workers try, but it is not enough of them for the number of patients. The owners should hire more people. Understaffed and overworked doesn't equate to quality care.

Please do not be fooled by the beautiful and clean rooms. The nursing and medical care at Signature a horrible experience for my 90 year old grandmother. She went in for "rehab" and left in worse shape. There is an extreme lack of communication. I voiced several concerns and asked to be contacted several times. The only person who called me was the finance staff asking how I was going to pay after her medicare coverage. In fact she called 3 times but I never received a call about my grandmother's medical care. I asked to speak to the nurse practitioner several times and finally had to go find her in her office. She had the worse dismissive attitude of a provider I have ever encountered. I was concerned about my grandmother's low heart rate while there and when I asked the nurse about it. His response was "I haven't assessed her all day" and still didn't seem to have any concern. I expressed concern about her medications and left messages again for someone to call and discuss. Never done. When I finally asked Christine, she replied "she didn't know they were being held and hadn't look at the hospital discharge records". After the first 5 days of no communication I had to ask for a family meeting to find out what the plan and goals were. No one really knew. She received physical therapy in the mornings and the rest of the days left in her room. I personally had to come on a daily basis to help her with bathing, dressing, and take her outside or provide interaction with her. She had a significant cognitive decline while there due to lack of interactions or encouragement from the staff. They seemed to be more interested in socializing at the desk. Never even saw the case manager. The day I went to see her she was nearly passed out in her room with a low heart rate again (never addressed from my previous concerns reported). I had to tell them to call a ambulance to go to the ER. I asked yhe tech to take her blood pressure and the technician gave me the vital sign machine to take it and left. The nursing and provider care there is the worst. We were told at admission she would have a lot of interaction, participation, and a care plan with goals. All LIES. It is a beautiful room but that is just a way to cover up the lack of quality care not provided here. Since we left to go to the ER no one there had even called to check on her or to see if she was still alive or dead. Again the financial person DID call and asked if we wanted to pay $400 a day to hold her bed while in the hospital. I moved her stuff out of room and made 3 trips to carry her stuff out. No one ever said a word while there !! This place is all about selling you LIES, taking your money or Medicare money, and then leaving you to take care of yourself. The nurses had bad attitudes if questions were asked. The director was unhelpful and told us her declining state was "normal" and that's it. I will be contacting the state and Medicare about the poor medical care given here. My grandmother came there for rehab and left there with a worse decline, walking with a cane, low pulse, high blood pressure, worsening of memory, and unable to do anything more than when admitted. In addition, the food is awful. If you admit a family member there I suggest you provide your own medical care because you will receive very little here. My grandmother's husband died here in June too.
